Reminder
Students must submit FOUR online forms in order to schedule a recital; the Event Request form, the Program form, the Tech form, and the COVID Protocol form.
The Event Request form reserves a date on the master calendar, and must be completed several no later than census day of the semester that the recital is performed.
The Program form lists the program and assisting performers, and is used to obtain the consent of faculty for the content and date of the recital and the participation of student performers. This form should be submitted not less than 30 days prior to your recital.
The Tech form notifies the tech staff of special needs (e.g., pianos, chairs, stands) and arranges for recording the recital. This form should be submitted not less than 30 days prior to your recital.
The COVID Protocol form is used to ensure understanding of Arizona Arts and Fred Fox School of Music COVID-19 policies and procedures. This is also the form used to request exceptions from these policies. This form should be submitted not less than 30 days prior to your recital.
Failure to meet these deadlines may lead to the cancellation of your recital.
